Distribution

Scott Reef off northwest Western Australia. Elsewhere the species occurs in the west-central Pacific: Indonesia and the Philippines, east to French Polynesia. Inhabits shallow coral and rocky reefs.

Etymology

The specific name is from the Latin formosus (= beautiful), in reference to the beautiful colour pattern of this species.
